Population composition and locality change
The under-75 denominator declined 2.5%, while total catchment population was essentially unchanged

The change primarily reflects an aging population, not broad catchment population loss.

From 2010 to 2024, the under-75 population fell by 22,447, while the total population fell by only 431. The population age 75 and older grew by 22,016.

Largest under-75 decreases

Wise, Tazewell, Buchanan, Lee, and Russell counties recorded the largest decreases. Washington County, Tennessee recorded the largest offsetting increase.

Why the denominator matters

A smaller under-75 denominator raises the population-based YPLL rate when YPLL is otherwise unchanged. The report therefore presents both total YPLL75 and the observed rate.

County changes may reflect migration, births and deaths, and residents aging into or out of the under-75 denominator. The locality table reconciles to the catchment totals shown above.

Series note: 2010-2019 uses Census intercensal county age-detail estimates; 2020-2024 uses the Census Vintage 2024 county age-detail estimates. Values are July 1 estimates.

2 · Latest-year change

The 2024 decline was driven primarily by lower accident-related YPLL.

The total decline was partly offset by increases in perinatal and cardiovascular YPLL75.

5 · Infant YPLL75

Infant deaths declined over the long term, but increased in 2024.

Each reported death under age 1 contributes 74.5 YPLL75 under the midpoint method. The chart shows deaths once; the adjacent facts show the corresponding YPLL75.

Infant data, YPLL75 calculation, and rate definition
Source pull, midpoint weight, completeness, and why this is not an infant mortality rate

Source

CDC WONDER all-cause pull grouped by ten-year age group and year, without the UCD/ICD-10 113 cause breakdown. All 15 annual under-one cells from 2010 through 2024 are numeric.

YPLL75 calculation

The under-one midpoint is 0.5. Each reported death contributes 75 - 0.5 = 74.5 YPLL75.

Rate distinction

These are death counts and calculated YPLL75, not an infant mortality rate. A rate requires live-birth denominators.

6 · Sex patterns

Male YPLL75 remained higher throughout the period, while both groups improved after 2021.

The CDC source reports Male and Female categories. This section shows YPLL75 from exact, non-suppressed rows; it does not compare sex-specific population-adjusted risk.

How the comparison was calculated

All-cause totals
CDC pulls grouped by rural/urban classification and ten-year age group.
Cause comparison
Exact numeric rows from the non-overlapping ICD-10 113 broad frame; suppressed cells remain excluded.
Rate denominator
Annual Census populations under age 75, summed across localities and years to create person-years.
YPLL75
Deaths multiplied by the report midpoint weight for each age group, using reference age 75.
Why these totals differ from the headline total

The headline is built from annual, cause-specific exact cells. This section uses broader pooled all-cause age extracts, which recover values hidden in more granular tables. The totals answer different questions and should not be added or expected to reconcile exactly.

8 · State and National Benchmark

SWVA + NETN remained above the Tennessee, Virginia, and U.S. YPLL75 benchmarks in every year.

Observed YPLL75 rates make differently sized geographies comparable. State and national values provide context, not independent control groups or county-level risk estimates.

Questions the benchmark answers first

Selected-period position, persistence, mortality volume, and age-at-death context.

Crude observed rates · not age-adjusted
How far apart were the pooled rates?
10,409 per 100K locally

+6.9% versus Tennessee, +56.5% versus Virginia, and +41.2% versus the U.S.

Is the difference persistent?
15 of 15 years

Across 2010-2024, the local annual rate exceeded Tennessee, Virginia, and the U.S. every year. For the selected period, the pooled rate was +6.9% versus TN, +56.5% versus VA, and +41.2% versus the U.S.

More deaths, or earlier deaths?
709 pooled exact deaths per 100K

TN 574 · VA 399 · U.S. 428. Local YPLL/death was lower than all three benchmarks, so the higher rate aligns more with mortality volume than earlier average age at death.

Selected-period rate versus 2010
+16.1% locally

Tennessee +11.7% · Virginia +8.5% · U.S. +8.4%.

Observed YPLL75 rate trend

All four geographies peaked in 2021 and improved afterward; the local rate remains above all three benchmarks.

What the comparison adds

The local observed rate was 6.9% above Tennessee, 56.5% above Virginia, and 41.2% above the U.S. Yet local YPLL75 per death was lower than all three benchmark values, indicating that the exact under-75 deaths were concentrated at somewhat older ages.

Recovery from the 2021 peak

Local -24.9% · Tennessee -26.1% · Virginia -22.9% · United States -23.1%

All four rates fell substantially after the shared 2021 peak.

2024 momentum

Local -5.0% · Tennessee -8.5% · Virginia -8.4% · United States -7.3%

All four rates declined in 2024; the three benchmarks declined faster than the local rate.

Under-75 population since 2010

Local -2.5% · Tennessee +12.1% · Virginia +7.7% · United States +8.3%

A smaller local denominator amplifies the rate increase when absolute YPLL remains high.

Reference point: 2024 rates were essentially back to their 2019 levels (Local -0.1% · Tennessee +0.1% · Virginia -0.1% · United States -0.7%), but the longstanding local-benchmark gap remained.

Tennessee, Virginia, and the United States matched exactly across 2018, 2019, and 2020 in both CDC source series. Deaths, YPLL75, population, suppression status, and every year-age-cause key reconciled before the series were joined.

These are descriptive benchmarks. Tennessee, Virginia, and the United States include the SWVA + NETN localities and are not statistically independent comparison groups.
Rates are observed crude YPLL75 rates for residents under age 75, not age-adjusted rates. Age-specific comparisons are provided to show population-pattern differences.
Suppressed CDC cells are retained in the audit and excluded from exact deaths and YPLL75; no suppressed count is estimated or treated as zero.
Because smaller geographies generally have more suppressed cells, SWVA + NETN exact death and YPLL75 totals may be more understated than state or national totals.
State and national raw totals are not compared directly with the coverage-area total because their populations are much larger.
The 0-4 comparison band combines the CDC <1 and 1-4 mortality rows to match the available catchment population denominator.
Calculation: YPLL75 = exact deaths × (75 − age-group midpoint). The same 53-code additive broad frame is used for SWVA + NETN, Tennessee, Virginia, and the United States. Zero rows remain zero; suppressed rows are excluded and never imputed.

The report shows total YPLL75 plus the catchment-wide actual YPLL75 rate per 100,000 residents under age 75. It is appropriate for identifying YPLL75 drivers and planning priorities. County-to-county comparison would still require a county-specific, suppression-safe rate analysis.
Zero and suppressed rows remain visible in the CDC source audit. Exact YPLL totals use positive numeric deaths only, so suppressed cells are not silently treated as zero.

Source use

Mortality, death, cause, age, and YPLL75 values are generated from the CDC source pulls; Census estimates provide population denominators for the observed rate. No external comparison report supplies displayed results.

Reference age

Midpoints

YPLL formula

YPLL75 = deaths x (75 - age-group midpoint).

Population context

Annual catchment resident population denominators are from the U.S. Census Bureau. The report shows the actual YPLL75 rate per 100,000 residents under age 75 alongside total YPLL75.

Rate caveat

The rate describes the combined SWVA + NETN coverage area. County-level comparison requires a county-specific, suppression-safe rate analysis.

Additive frame

Year, age, cause, trend, and projection views use positive numeric rows in the non-overlapping broad ICD-10 113-cause frame.

Detail views

Cancer, heart, accident, despair, and COVID views are drilldowns; parent/detail rows should not be added together.

Zero and suppressed rows

The raw CDC pull includes 10,699 zero-death rows and 4,740 suppressed rows. Zero rows are retained for audit but do not change YPLL totals; suppressed rows are retained and flagged but excluded from exact death/YPLL totals because CDC does not provide the count.

ICD-10 113 drilldown

Broad-frame totals use the selected non-overlapping parent rows only. Detail rows shown beneath a parent are drilldown evidence from the same CDC pull and are not added to the broad-frame totals.

CDC query notes

The downloadable source-data workbook keeps each CDC pull on a separate worksheet. Source footer/query notes are retained with the corresponding pull when available, and the package includes a source index and data dictionary.

Scenario planning

The 2030 paths are illustrative planning references: a 2010-2019 long-run trajectory, return to the 2010-2019 average, and a 10% reduction target from the 2024 actual. They are not forecasts or confidence intervals.
